Requirements
- Any two (2) valid identification cards with signature
- Notarized Waiver of Rights
- Proof of house and/or lot ownership. A copy of the absolute deed of sale in lieu of title (if a change of name is due to newly acquired or bought property)
- One (1) pc. 2×2 ID picture
- Authorization from house and lot owner, if tenant only
- A copy of the death certificate of the deceased consumer
- Mayor’s Permit if business establishments;
- SEC Registration for Corporations Signed Water Service Contract
