 Signed New Water Service Connection Application Information Sheet

 Bill for the month of March and April are based on ACTUAL CONSUMPTION
 50% of bill is due on MAY 30, 2020; remaining 50% is due on JUNE 30, 2020.
 No disconnection of service lines until further notice.
 Kindly DISREGARD the penalty/surcharge indicated on your bill. We will not impose penalty until further notice.
